Self Appraisal Review

For the organization that wants employees to perform a self-appraisal, this brief form allows the employee to do a self-evaluation to identify strengths, weaknesses and what the supervisor needs to do to help further the chances of success.  There is no score for this appraisal.  It is purely a development tool.
