Motor Vehicle Report (MVR) A report from the Department of Motor Vehicle Registration. An excellent tool to help determine if the applicant is a "responsible person," and how outside behavior might impact attendance and health care claims. A necessity for those who drive company vehicles or those who drive their own vehicles on company business.

Criminal Record Search (CRS) Determines felony and/or misdemeanor records. A vital report for anyone hiring people who will work with money, company records, drugs, valuable inventory, or come in contact with the public. Helps avoid legal liability of Negligent Hiring. You may choose to seach by city, county, statewide, or Federal Court Records. The cost is based on the depth nature of a search. A quote for the search can be given at the time of request. 1. Have the applicant read and sign the "Permissible Purpose Background Check Authorization". We are unable to check the applicant's background until the applicant signs the release.
2. The manager completes the bottom portion of the authorization form and provides the company name, the requesting manager's name and the type of background checks requested.
3. FAX the form to HR ENTERPRISE, INC. at (502)896-0033. If you prefer you may mail the form to HR ENTERPRISE, INC., P.O. Box 6507, Louisville, KY 40206-0507.
4. When the background check is complete you will receive a written report of the results in the mail. You may request the report faxed to you and/or a verbal report given over the phone.
